yang ini 403 terlihat tanpa harus buat files lagi
ErrorDocument 403 "403 mas.. mbak.. 4.0.3 + 4.0.10 = Kacian deh lu.."

ditambah dengan ban ip dkk yg jadinya 403 juga..
order allow,deny allow from all |
ok kalau httaccess ndak jalan check di httpd.conf mau dixampp, nginx atau di apache atau server config lainnya

cari itu.. ganti dari None ke All supaya htaccess jalan
